I suspect that's the known issue due to changes on the mathjax server. You either need to upgrade your install to a later revision or follow the instructions below:
MathJax changed their MathJax CDN hosting. Specifically they changed the https:// address we use in Xerte Online Toolkits. You can read more about that change here: www.mathjax.org/changes-to-the-mathjax-cdn/
There are 3 files in the Xerte install which point to the now broken source.
The 2.1 distribution, and 2.1 update packages are fixed.
You can also patch your installation:
These are the files that need fixing:

Subject: [Xerte] Problem with navigation icons in XoT objects
Recently I noticed that there accurred a problem with the navigation links under the objects created in XoT.
I will add a screen shot that will clarify what I mean. As you can see, the navigation icons are missing in the view.
This behaviour occurs in both the XoT webinterface as in the Chamilo learning environment where we import the scorm objects created in XoT.
I tested this on multiple pc's with multiple accounts and multiple browsers. All show the same missing navigation links.
Can anyone help me troubleshoot this problem?
