They look like standard form templates.

Do you get an error when you import?

[Also, if those are OER - as in Open Educational Resources then I'd add CC
license info to the page. You might also want to submit these OER via the
XOT syndication feed to Xpert (http://xpert.nottingham.ac.uk)]
