$debug doesn't appear in /xerte/config.php. A file search for "$debug" in the XOT installation finds:

C:\xampp\htdocs\xertetoolkits\LTI\ims-lti\lti_util.php
C:\xampp\htdocs\xertetoolkits\website_code\php\dUnzip2.inc.php
  C:\xampp\htdocs\xertetoolkits\website_code\php\management\get_templates_for_user.php

None of which are relevant to enabling PHP errors. As Mike's running XOT off a hosting account he'll not have access to php.ini to turn on error reporting. Error reporting can be enabled in script easily enough, but that assumes a) familiarity with scripting, and b) knowing which script(s) to put the code into.

Error 500 is classically unhelpful, as it just refers to "Internal server error" which could be caused by pretty much any script server-side.

It seems to be throwing a 500 error, which suggests a PHP issue.
If you edit config.php (i think) look for $debug = false, and set it to true

Then you'll get PHP errors.

The Xerte installation should be at http://camchem.net/hrxerte but I have an extra folder, so you have to open xertetoolkits_2.1 from the index page.
