One last thing to try... create a new lo, add an embed content page and embed your Google form code or just use the link - does that work? e.g. http://mitchellmedia.co.uk/xot/play_45

Does it work if you add just the link in the embed content page rather than the embed code? e.g. try 
https://docs.google.com/spreadsheet/viewform?formkey=dDNNS2prOE1Kdng4QzhVcXRZNUhMWWc6MQ
in the embed content page. Works on my installation.
Also just to test try temporarily moving the page to the top of the tree e.g. so it's the 1st or 2nd page - does it work now?

It's not up on the website yet, try to finalise it so that I can add it on our webpages. 

The link is http://app1.its.bbk.ac.uk/xerte/play.php?template_id=303 (feedback is the last page) The actual form is also linked on page no 12 but I would prefer have it embeded.
