This should be in the zip:

my_template\Example.rlt 
my_template\template.xwd 
my_template\template.xml 
my_template\common\* 
my_template\media\* 
my_template\models\* 
my_template\thumbs\*

The .xwd and the .xml file HAVE to be called template.xwd and template.xml, and they have to be in a folder in the zip ('my_template' in the example above)
