This is a bit of a wild guess, but what kind of permissions does the 
mysql user have?

It looks like XOT can connect to mysql, but it can't write/insert records.

Also, you're on Linux right? Could you try and switch on 'developer' 
mode in config.php by setting '$development=true' around line 26?

Hopefully you see an error message immediately, or after inspecting the 
/tmp/debug.log file.
