Sounds like a write fail on the root folder to me?
Hows does database.txt look in the setup folder?
If all five variables are set then you could copy that text into database.php manually.

I'm sure Pat will advise further but what is supposed to happen is that values you add to the form during setup are captured and added to database.php and I've known some installations to miss a value in that process which causes errors. In fact I think database.php gets created dynamically during the setup e.g. it doesn't exist as database.php to begin with. From memory I think it might be in the install files as databse.txt. But I've never known that file to be completely blank so not sure if that points to permissions or a wider issue. Have you checked the permissions against those detailed in the installation guide?
