[Xerte] Xerte Toolkits - Insert Button Enables Briefly on Load then Disables

Hi there,

I have installed Xerte Toolkits and have run into an interesting problem.

Upon launching a course to edit it the Insert and other buttons are active
for a brief instant and then become disabled.

Any idea what I have done wrong?
