Well, obviously (?!) I wasn't the one trying to create the LO.
Whoever was, had uploading an FLV file and then (if memory serves me 
correctly) found that it didn't render on an iPad.

(I think it was a case of them doing a straight migration from using 
flash-based LOs to the shiny new HTML5 variant...)
